Position Purpose
Under the direction of the facility administrator and/or AL operator, the Resident Care Coordinator is to ensure the provision of quality nursing care on a 24 hour basis to the residents of this assisted living facility in accordance with state and local regulations. The Resident Care Coordinator is a member of a team and is responsible for coordinating aspects of the team’s work.
Key Responsibilities/Duties
Coordinates admissions, including supervising move-ins, to ensure all accommodation is as desired and care is immediately implemented based on the residents’ Functional Capacity Screen (FCS) and Negotiated Service Agreement (NSA).
Clarifies medication orders with the primary care provider.
Provides daily audits of medication orders and medication carts.
Under the guidance of the AL Operator, performs the FCS, general assessments, orders medication from dispensing pharmacies and coordinates other impletion of all other physician orders.
Coordinates care planning with home health agencies and hospice agencies working on-site to ensure all contracted agencies work within the community’s policies and procedures.
Functions as a liaison between the residents, their families, and staff responding to inquiries and providing information on clinical care and services and may screen prospective residents to assure that this facility can provide the care needed.
Supervises the medication room and reviews physician orders to ensure accuracy and compliance.
Coordinates provision of special needs, requests, and complaints including verification of follow-through.
Monitors the quality of care provided to residents in the AL Memory Care and assist in the assessment process for admission to and exit from assisted living.
