Job Summary

The Nutrition Counselor provides nutrition care and food service support within a long-term care setting under the supervision of a Registered Dietitian Nutritionist. This role focuses on supporting resident nutritional health, monitoring dietary intake, ensuring compliance with therapeutic diets, and maintaining high standards of food safety and quality to enhance residents’ quality of life.

Key Responsibilities

Resident Nutrition Care

Conduct nutrition screenings and collect data on residents’ dietary needs, preferences, and risks

Monitor meal intake, weight changes, and hydration status

Assist in implementing and monitoring individualized nutrition care plans

Report changes in residents’ nutritional status to the RDN and interdisciplinary care team

Participate in care conferences as needed

Dietary Compliance & Meal Service

Ensure meals and snacks meet therapeutic and texture requirements

Perform tray audits and meal rounds to verify accuracy, portion sizes, and diet compliance

Collaborate with interdisciplinary team to address feeding concerns and adaptive needs

Food Safety & Quality Assurance

Monitor adherence to food safety, sanitation, and infection control standards

Assist in compliance with federal, state, and local regulations (e.g., CMS guidelines)

Participate in quality improvement and audit activities

Report and address food service concerns or deficiencies

Resident Engagement & Education

Provide basic nutrition education to residents and families as appropriate

Promote resident satisfaction by incorporating food preferences and cultural needs

Assist with menu planning that reflects resident choices and nutritional adequacy

Administrative & Operational Support

Maintain accurate and timely documentation in medical records

Assist with menu review, nutrient analysis, and production sheets

Train and support dietary aides as needed
