Hospice Social Worker

Job Description

As a Hospice Social Worker, you will not just complete visits. You will guide patients and families through one of the most important transitions of their lives.

This role is not just about resources and documentation. It is about presence, honesty, and helping families navigate what comes next with dignity.

You will support patients and families through end of life care while also providing bereavement support that continues after the loss. Your work will directly impact how families experience one of the most difficult times in their lives.

What You Will Do

• Provide emotional and psychosocial support to patients and families
• Guide families through end of life decisions with clarity and compassion
• Provide ongoing bereavement support before and after loss
• Work with hospice volunteers to support patient and family needs
• Complete assessments, care planning, and documentation in compliance with hospice regulations
• Collaborate with an interdisciplinary team focused on patient centered care

What We Are Looking For

• Masters Degree in Social Work (MSW) required
• Hospice or healthcare experience preferred
• Strong communication and documentation skills
• Ability to navigate difficult conversations with confidence and empathy

• Annual salary range 55,000 to 65,000 based on experience
