Role Description
We are seeking a compassionate and dedicated Social Worker for a PRN, part-time or full-time, on-site position in West Des Moines, IA. The Social Worker will provide psychosocial support to patients and families by assessing needs, offering emotional counseling, and assisting with coping strategies during the end-of-life journey. Responsibilities include care planning, resource coordination, crisis intervention, and advocacy while collaborating with the interdisciplinary team to ensure comprehensive patient care. The role requires applying a deep understanding of cultural and social dynamics to address individual and family needs during challenging times.
Qualifications
- Strong clinical skills, including psychosocial assessments, care planning, and crisis intervention.
- Knowledge of community resources, social service systems, and family dynamics to assist patients and caregivers effectively.
- Compassionate counseling and communication abilities to support patients and their families during emotionally demanding situations.
- Proficiency in cultural competence and sensitivity to diverse backgrounds and experiences.
- Licensed Master Social Worker (LMSW) or equivalent credentials, with a preference for candidates with experience in hospice or palliative care.
- Exceptional organizational and documentation skills with the ability to work collaboratively in an interdisciplinary team environment.
- Strong ethical decision-making and problem-solving skills, demonstrating commitment to patient dignity and care.
- Familiarity with CMS guidelines and hospice care protocols is an advantage.
- Ability to travel within the West Des Moines area as required and provide on-site support to patients and families.
