WCS Healthcare is seeking an experienced and compassionate Lead RN Care Coordinator to oversee the delivery of comprehensive health services for children in New Haven, CT.
Job Function:
- Day Shift.
- 8:30 AM – 5:00 PM.
- Oversee and coordinate healthcare services for children, ensuring timely and appropriate medical, dental, and behavioral health support
- Supervise and mentor care coordination staff, providing day-to-day guidance and oversight
- Develop and deliver staff training on healthcare protocols, public health practices, and compliance standards
- Coordinate interpretation and translation services to ensure equitable access to care
- Support care transitions by coordinating post-program or post-release healthcare services
- Collaborate with healthcare providers, agencies, and community resources to ensure continuity of care
- Monitor compliance with healthcare regulations and organizational policies
- Maintain accurate, timely, and compliant medical documentation
- Participate in interdisciplinary meetings to support holistic care planning
Job Requirements:
- Active Registered Nurse (RN) license in Connecticut
- Bachelor’s degree in Nursing or higher
- Minimum of 2+ years of clinical experience (pediatrics, community health, or care coordination strongly preferred)
- Strong knowledge of care management principles and community resources
- Excellent communication and organizational skills
- Ability to work independently while collaborating within a team environment
- Prior supervisory or leadership experience in a healthcare environment
- Bilingual in English and Spanish (required)
