The Community Liaison is responsible for developing, managing, and growing referral relationships within the community to support patient access and service utilization for Homedica House Calls. This role serves as a key representative of the organization, educating healthcare providers, facilities, and community partners on available services, including in-home primary care, telehealth, chronic care management, and specialty programs.
QUALIFICATIONS
- 3 years of experience in healthcare marketing, business development, or community outreach preferred
- Experience in home health, hospice, primary care, or post-acute care strongly preferred
- Strong organizational skills with the ability to prioritize tasks in a fast-paced, field-based environment
- Demonstrated ability to handle confidential and sensitive information with professionalism and discretion
- Strong verbal and written communication skills with the ability to engage interdisciplinary teams and external partners
- Proficiency in Microsoft Office (Excel, Outlook, Teams) and general computer systems; experience with CRM or EMR systems preferred
- Ability to work independently while maintaining accountability to team goals and performance metrics
- Strong problem-solving skills and attention to detail
- Ability to build and maintain professional relationships with referral sources and community partners
- Comfortable presenting information to individuals and groups in both formal and informal settings
- Understanding of healthcare payer models, including Medicare and Medicaid Managed Care, preferred
- Valid driver’s license and reliable transportation required for local travel
