Job Title: Field Care Manager (South Jersey & North Jersey Regions)

Position Overview

We are seeking dedicated and reliable Field Care Managers to conduct in-person client visits throughout the South Jersey and North Jersey regions. This role is ideal for individuals who are organized, detail-oriented, and comfortable working independently in the field while maintaining strong communication with clients and internal teams.

Key Responsibilities

• Conduct face-to-face visits with assigned clients in the community
• Document each visit thoroughly, including client status, needs, and any changes in condition
• Provide timely updates and reports to the care team or management
• Ensure all required documentation is completed accurately and submitted promptly
• Obtain client signatures to confirm completed visits
• Maintain professional and compassionate interactions with all clients
• Collaborate with another Care Manager per case to ensure continuity and quality of care
• Adhere to company policies, compliance standards, and confidentiality requirements

Compensation

• $50 per completed case/visit
• Compensation includes travel and mileage expenses
• This is a 1099 position

Qualifications

• Bachelor’s Degree (BA) in Social Services, Psychology, or a related field required
• Previous experience in healthcare, social services, case management, or a related field preferred
• Strong documentation and organizational skills
• Ability to work independently and manage a flexible schedule
• Reliable transportation and valid driver’s license
• Comfortable traveling within assigned region (South Jersey or North Jersey)
• Excellent communication and interpersonal skills

Requirements

• Must be able to pass all required background checks due to working with vulnerable populations
• Must be willing to accept a minimum of 15 cases
• Must comply with all applicable state and organizational regulations

Work Environment

• Field-based role requiring travel within designated region
• Flexible scheduling based on client needs and visit assignments
