LSSI is hiring a full-time Intact Family Recovery Counselor Case Manager, Monday through Friday. Responsible for providing intensive, home-based clinical and case management services to women referred by DCFS who have given birth to a substance-exposed infant and for other referrals made of families with significant drug usage indicating risk of harm to affected children.

Essential Functions:
  • Provide case management, outreach, and aftercare services to clients and their families; typical caseload is 15-20 clients.
  • Provide services including but now limited to substance abuse assessments, treatment planning, referral/linkage, crisis intervention, drug screens, and other advocacy services.
  • Facilitate supportive treatment invention to maintain or enhance client’s capacity for independent functioning.
  • Facilitate client’s admission to in-patient substance abuse treatment or linkage to out-patient treatment as needed.
  • Intervene using a holistic approach to change attitudes, beliefs, and behavior of clients in an effort to improve client and family functioning.
  • Work in collaboration with DCFS, Juvenile Court and other community service agencies.
  • Complete and submit all necessary reports and documentation accurately and on time as required by agency, regulatory bodies, and funding sources.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
Position Qualifications:
  • High school diploma or equivalent required.
  • Bachelor’s degree in social work or related human service field preferred.
  • At time of hiring, must meet certification requirements for CARS (Certified Assessment & Referral Specialist).
  • CADC (Certified Alcohol and Drug Counselor) required or must obtain no later than two (2) years from date of hire.
  • Demonstrated ability to relate to others and influence people.
  • Demonstrated ability to communicate in a clear, comprehensible manner, verbally and written.
  • Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
  • Background check clearance, both criminal and CANTS required.
  • Valid driver’s license, in good standing for the state of residency required.
  • Valid IL statutory minimum liability insurance coverage, bodily injury, and property damage required.
  • Access to reliable transportation required.
