Job Summary
Prima Care is seeking a dedicated and compassionate Behavioral Health Care Manager (CoCM) to join our integrated care team. This role is a key component of our Collaborative Care Model (CoCM), working directly with patients, primary care providers, and a psychiatric consultant to deliver evidence-based behavioral health services within the primary care setting.
The Behavioral Health Care Manager serves as the primary behavioral health contact for patients, providing ongoing support, brief interventions, symptom monitoring, and care coordination. This position plays a critical role in improving patient outcomes through a structured, team-based approach to mental health care.
Key Responsibilities
Clinical Care & Patient Engagement
- Provide behavioral health assessment and care management services, including:
- Administration of validated screening tools (e.g., PHQ-9, GAD-7)
- Development and implementation of individualized care plans
- Ongoing monitoring and adjustment of care plans based on patient progress
- Delivery of brief, evidence-based psychosocial interventions (e.g., CBT-informed strategies, behavioral activation, motivational interviewing)
- Maintain ongoing therapeutic relationships with patients through regular follow-up (telehealth and/or in-person)
Collaborative Care Model (CoCM) Activities
- Present patient cases regularly to the psychiatric consultant and implement treatment recommendations
- Collaborate closely with the primary care provider (billing practitioner) to support integrated treatment planning
- Serve as the liaison between patient, PCP, and psychiatric consultant to ensure coordinated care
- Participate in interdisciplinary case reviews and care team meetings
Care Coordination & Program Management
- Maintain accurate and up-to-date patient registry/tracking systems
- Monitor patient outcomes and ensure timely follow-up and engagement
- Identify patients not improving and escalate care appropriately
- Support patient access to additional behavioral health resources when clinically indicated
Documentation & Compliance
- Document all patient interactions in the electronic health record in accordance with CoCM billing and regulatory requirements
- Ensure compliance with CMS CoCM guidelines and program standards
Qualifications
Required
- Master’s degree in Social Work or related behavioral health field
- Active Massachusetts licensure:
- LICSW or LMHC
- Experience in behavioral health, care management, or integrated care settings
- Strong knowledge of evidence-based treatment approaches, including:
- Cognitive Behavioral Therapy (CBT)
- Behavioral Activation
- Motivational Interviewing
- Problem-Solving Therapy (DBT experience a plus)
- Experience administering and interpreting validated screening tools (PHQ-9, GAD-7)
Preferred
- Experience in Collaborative Care Model (CoCM) or integrated primary care settings
- Familiarity with registry-based population health management
- Experience working with diverse patient populations

Pay: $51,353.75 - $61,845.38 per year
