Social Worker

Join our dedicated team as a Social Worker and make a meaningful impact on individuals recuperating from a short-term post-acute hospital stays and those residing for traditional long-term care or memory care support.

In this vital role, you will provide compassionate support, conduct thorough assessments, and develop personalized care plans to promote well-being and including with family supports.

Primary Responsibilities:

• Conduct comprehensive admission evaluations and on-going assessments to identify resident/patient needs and develop individualized care plans.
• Complete regular sections of the MDS and related assessments.
• Coordinate with multidisciplinary teams to ensure seamless case management, treatment planning, and service delivery.
• Document accurately within electronic medical records (Point Click Care) while adhering to HIPAA regulations for confidentiality.
• Plan and coordinate with home and community-based services for patient discharge needs and plan of care supports including post-stay follow-up.
• Actively participate in organizational leadership and quality assurance meetings.

Preferred Candidates should have MDS 3.0 and Point Click Care proficiency, knowledge of CMS and MA DPH skilled-nursing and long-term care regulations, and excellent communication skills for patient assessment, patient care planning, and interdisciplinary collaboration.

Pay: $70,720.00 - $91,520.00 per year

• Skilled nursing facility: 3 years (Preferred)
