CATHOLIC CHARITIES OF BROOME COUNTYResidential Services – Certified Apartments ProgramCare Manager - Level 2 GRADE  8+ $20.00- $21.65                                                              FLSA (Non-Exempt)I.     QUALIFICATIONS:Education/Experience Bachelor’s degree in Human Services preferredAssociates degree in the Human Service field plus 3 years’ experience ORIn lieu of a degree; Minimum of 5 years’ experience working with individuals with a mental health diagnosisSkills Good organizational and time management skillsGood oral/written communication skillsGood interpersonal skillsKnowledge of community agencies and resourcesAbility to work with service providersFamiliar with computer programs (e.g. Word, Excel)The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs. Ability to legally operate a vehicle in NY StateII.        RESPONSIBLE TO:            SupervisorIII.      MAJOR FUNCTIONS: Conduct initial and on-going assessmentsConduct care plan meetings with all relevant service providersAct as a linkage to community services including medical, behavioral, residential, benefit services etc.Promote health and wellness activitiesMust utilize software technology pertinent to programMay need to participate in afterhours/weekend on call coverageMaintain required documentationEnsure that a person-centered planning process is employed in delivery of servicesLink and connect to natural supports and community resourcesProvide crisis intervention as necessaryComplete required reports/notes in a timely and professional mannersComply with all applicable regulationsEnsure that individual rights are protectedAdvocate for individualsMaintain documentation necessary to substantiate billing/fundingParticipate in and successfully complete all required staff training and development activitiesRepresent the agency and program in the community and with other service providersRegular and substantial contact with individuals receiving servicesParticipate in team meetingsPerform other job-related activities as directed by supervisorUnderstand and follow all program policies and proceduresCatholic Charities of Broome County is an equal opportunity employer.
