Job Summary:

As a Social Worker, you will play a vital role in our social services department, working closely with healthcare professionals to address the social needs of our patients. You will assist with assessments, care planning, and social service records, as well as participate in discharge planning and community outreach initiatives.

Responsibilities:

• Participate as a member of the team to address Social Service sections of MDS and care plans

• Maintain social service records and complete assessments of new and current residents

• Assist with discharge planning and develop strategies for follow-up care

• Act as a liaison between the facility and the community

• Maintain accurate records and report on clients' status

Requirements:

• Valid NJ LSW

• At least 2 years' experience in healthcare, nursing facility preferred

• Master's degree in Social Work or a related field
