Job Overview
We are seeking a compassionate and dedicated In-home Geriatric Licensed Master Social Worker (LMSW) to provide comprehensive mental health and social support services directly within the homes of elderly clients. Responsibilities
- Conduct thorough patient assessments to evaluate mental health, cognitive status, social needs, and overall well-being using diagnostic tools aligned with ICD-9 and ICD-10 coding standards.
- Develop individualized care plans incorporating psychotherapy modalities such as cognitive behavioral therapy, psychodynamic therapy, grief counseling, and behavioral therapy tailored to geriatric patients.
- Provide direct support through individual and group counseling sessions focused on issues like depression, anxiety, PTSD, grief, chronic pain management, addiction recovery, gender issues counseling, and behavioral management.
- Utilize motivational interviewing techniques to foster patient engagement and promote positive behavioral changes.
- Coordinate with healthcare providers, family members, caregivers, and community resources to ensure comprehensive case management and continuity of care.
- Maintain accurate medical documentation adhering to HIPAA regulations, including detailed progress notes, treatment plans, and medical records using appropriate medical terminology and ICD coding.
- Respond effectively to crisis situations such as mental health emergencies or behavioral crises through crisis intervention strategies and crisis management protocols.
- Participate in clinical research initiatives related to behavioral health in geriatrics and contribute data collection efforts for quality improvement.
- Utilize telehealth platforms for remote consultations when appropriate, ensuring seamless patient care delivery across different settings.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution with valid licensure as an LMSW or equivalent.
- Proven experience working with elderly populations in home settings or outpatient clinics; hospital experience is highly valued.
- Strong knowledge of mental health counseling techniques including psychotherapy, psychopharmacology basics, addiction counseling, grief counseling, trauma-informed care (including PTSD), and behavior management strategies.
- Familiarity with ICD-9/ICD-10 coding systems for diagnostic evaluation and billing purposes.
- Experience with telehealth platforms and delivering services remotely is preferred.
- Excellent communication skills with the ability to work effectively with children & families when necessary; experience working with individuals with developmental disabilities or autism is a plus.
- Ability to handle crisis situations calmly and efficiently while maintaining client safety through crisis intervention protocols.
- Knowledge of medical terminology related to behavioral health conditions and chronic disease management in geriatrics.
