Provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation.

- Provides complete psychosocial assessments of patients and families.
- Provides counseling to individuals and families to help solve social and emotional problems.
- Coordinates community resources to address/meet identified needs.
- Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
- Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
- Organizes support groups.
- Documents objective, concise and pertinent clinical data in the medical record.
- Attends multidisciplinary clinics with the CF team
- Conducts regular screening for depression and anxiety
- Builds familiarity with and helps patients access CF-related resources
- Attends national and regional CF conferences yearly along with other team members
- Serve as a Mental Health Coordinator as defined by the CF foundation, helping patients locate mental health providers including therapists and psychiatrists for ongoing mental health needs
- Master’s Degree in Social Work (MSW) required.
- Social work experience in an acute, home care, or ambulatory setting preferred.
