Description

Position Summary: Responsible for planning, coordinating, and executing community-based outreach activities that promote access to care through the Rural Care Connect program. This role serves as a key liaison between Deborah Heart and Lung Center and rural communities, helping to connect individuals to needed healthcare services. Organizes and represents the organization at community events, health fairs, and partner sites, while supporting patient navigation and follow-up. This position requires frequent local travel within designated rural service areas and flexibility to work evenings and weekends to support community engagement efforts.

Experience

Required: Minimum 2 years’ experience in community outreach, event coordination, healthcare support, or a related field.

Preferred: Experience in healthcare, public health, or community-based programs. Prior experience organizing events, health fairs, or outreach initiatives. Familiarity with rural or underserved populations.


Education

Required: High School diploma or equivalent.

Preferred: Assoc. Degree in public health, healthcare administration, communications, social services, or related field.


License and Credentials

Required: Valid driver’s license with reliable transportation

Preferred: Basic Life Support Certification (BLS)


Skills

Required: Strong interpersonal and communication skills, with the ability to engage effectively with diverse populations. Excellent organizational and event coordination skills, with attention to detail and follow-through. Ability to work independently in community settings and collaboratively within a team environment. Cultural competence and sensitivity to the needs of rural and underserved populations. Basic proficiency with computers and data entry, including Microsoft Office and/or tracking systems. Ability to manage multiple priorities and adapt to changing schedules and environments. Professional demeanor with the ability to represent the organization positively in the community.

The minimum starting rate for this position is $19.11

When determining a team members base rate, several factors may be considered as applicable (e.g., years of recent relevant experience, education, credentials, and internal equity).
