Social Worker Bilingual (Spanish/English) – Skilled Nursing Facility

About the Role

The Phoenix Rehabilitation and Nursing Center is seeking a compassionate and motivated Bilingual Social Worker (Spanish/English) to join our interdisciplinary care team. This role plays a vital part in supporting residents and families with psychosocial needs, care planning, and discharge coordination in a skilled nursing environment.

The ideal candidate is empathetic, culturally competent, well-organized, and passionate about advocating for residents.

Social Worker Bilingual (Spanish/English) Key Responsibilities

  • Conduct psychosocial assessments and develop care plans

  • Provide counseling and emotional support to residents and families

  • Serve as liaison between residents, families, and interdisciplinary team

  • Facilitate discharge planning and coordinate community resources

  • Participate in admissions and resident orientation

  • Address resident and family concerns and grievances

  • Maintain accurate documentation in the EMR system

  • Participate in care conferences and family meetings

  • Ensure compliance with CMS and state regulations

Social Worker Bilingual (Spanish/English) Qualifications

  • Bachelor’s or Master’s degree in Social Work (BSW or MSW)

  • Bilingual in Spanish and English (required)

  • At least 1 year experience in a healthcare or long-term care setting preferred

  • Strong communication, organizational, and interpersonal skills

  • Knowledge of discharge planning and community resources

  • Experience with PointClickCare or similar EMR preferred

Salary: Up to $70,000 per year
(based on experience)
