Job Title: Licensed Master Social Worker (LMSW) – InpatientSpecialty: Social Services / Case ManagementShift: Day Shift – 5x8 Hours (09:00 AM – 05:00 PM)Schedule: Monday – Friday (Excludes Federal Holidays)Duration: 14 WeeksDesired Location: New York, NYJob Description:We are seeking a skilled Licensed Master Social Worker (LMSW) to support inpatient care teams with psychosocial assessments, discharge planning, and coordination of post-acute services. The ideal candidate will have strong hospital-based experience and the ability to manage a high-volume caseload.Key Responsibilities:Conduct comprehensive psychosocial assessments for patientsDevelop and implement discharge plans for inpatient populationsCoordinate post-discharge care referrals (e.g., home care, rehab, hospice)Manage a caseload of 25–30 patientsCollaborate with interdisciplinary teams including physicians, nurses, and case managersDocument patient interactions and plans in EMR systems (Epic preferred)Support patients and families through complex care transitionsKey Requirements:Master’s Degree in Social Work (MSW) – RequiredActive LMSW License (New York State) – RequiredStrong knowledge of psychosocial assessment and discharge planningExcellent communication and interpersonal skillsPreferred Qualifications:Inpatient hospital discharge planning experience (strongly preferred)Experience with: Hospice careHemodialysis patientsSubstance abuse casesGuardianship and ethical casesExperience with Epic or Allscripts EMR systemsFor more details reach at sthakur@navitashealth.com or Call / Text at 732 791 4807 - EXT 4807.
