A Home Health Intake Coordinator manages the initial patient referral process, acting as the key liaison between hospitals, physicians, families, and clinical staff to ensure a seamless start of care. They are responsible for receiving referrals, verifying insurance benefits, obtaining authorizations, and accurately entering patient data into Electronic Medical Records (EMR).
As a Intake Referral Coordinator, you will:
Receives and appropriately documents new referrals in a timely manner.
Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications.
Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
Provides effective communication to patients, staff members, other health care professionals and referral sources.
Responds to Agency and patient needs in a professional and creative manner.
Demonstrates commitment, professional growth and competency.
Participates in Agency-sponsored in-service training.
Participates in the Agency’s QAPI program.
Promotes the Agency philosophy and administrative policies to ensure quality of care.
Requirements
Minimum one year of experience in referral intake is preferred
Experience with third party payers, Medicare and Medicaid highly desirable
Medical Field background preferred, including familiarity with medical terminology
Excellent computer and customer service skills
LVN/LPN preferred
Valid Driver’s License
Valid Auto Insurance
HomeCare HomeBase experience preferred
