Baptist Medical Center is looking for an Care Manager to join our team in Jacksonville, FL. This is a Full-Time days opportunity located at Baptist Metro Square campus.

Care Manager, Job Responsibilities:

• High level screening, assessing and planning for an identified high risk group of patients in the ER, Critical Care and other services as needed.
• Collaborate with the attending, ancillary, professional and technical staff to coordinate patient care.
• Screen patient for admissions or discharge utilizing Utilization Review criteria.
• Participate in oversight of resources in critical care and emergency services area.

Experience Requirements:

• 3-5 Years Case Management Experience
• 3-5 Years Emergency Department Experience
• 3-5 Years Discharge Planning Experience
• 3-5 Years Critical Care Experience

Education & Credential Requirements:

• Bachelor of Science in Nursing required
• Master of Arts/Science preferred
• Basic Life Support (BLS) required
• Licensed Registered Nurse required
