Hospice Medical Social Worker (MSW/LCSW Preferred)

Schedule: Monday–Friday

Job Summary

We are seeking a compassionate and experienced Hospice Medical Social Worker to join our interdisciplinary hospice team. This role provides psychosocial assessments, counseling, and care coordination to hospice patients and their families, supporting emotional, social, financial, and environmental needs during end-of-life care.

Responsibilities

• Complete psychosocial assessments within 5 days of hospice admission
• Identify psychosocial needs and develop interventions for the plan of care
• Participate in Interdisciplinary Group (IDG) meetings
• Provide counseling, crisis intervention, and emotional support
• Document assessments and interventions in compliance with HIPAA and CMS regulations
• Collaborate with nurses, physicians, chaplains, and other team members

Qualifications

• Bachelor’s or Master’s degree in Social Work
• Minimum 1 year hospice social work experience (required)
• LSW or LCSW preferred
• Experience with counseling, patient assessments, and documentation
• Strong communication and interpersonal skills
