Harmony Place Monterey is a small outpatient mental health facility dedicated to providing comprehensive treatment for adults dealing with Mental Health challenges, Trauma/PTSD, Eating Disorders, Chemical Dependency, and Sexual Disorders through our Intensive Day Treatment Program.
In this dynamic position, the Behavioral Health Care Coordinator will work alongside with a multi-disciplinary team of highly trained and experienced professionals. Most importantly, you will be the first connection the client experiences! Additionally, this position will work closely with the Director of Operations to perform quality assurance reviews including chart audits.

Job Responsibilities:
- Assist in coordinating program admissions with Director of Admissions and Senior Therapist.
- Coordinate paperwork prior to admission.
- Verify completion of all necessary forms, consents, and other documents in Electronic Health Records (EHR).
- Follow-up requests for release of previous treatment records including discharge summaries.
- Conduct orientation for new clients.
- Coordinates new client Admissions and communicates arrival information to client and team
- Ensures pre-admission forms are sent to clients and collected prior to admission
- Conducts UAs upon admit and for all clients in Chemical Dependency track per P&P requirements
- Weights and Vitals upon admission for all for admits and all Eating Disorder Track clients per P&P.
- Monitor floor during client breaks.
- Assist with coordination and completion of client assigned assessments upon admission, at monthly intervals, and upon discharge.
- Assist in facilitating the discharge process - complete medication regimen, completing discharge plan, including aftercare appointments, and collection client satisfaction surveys.
- Coordinate the Doctor’s Schedule.
- Audit charts in alignment with identified performance improvement goals relative to accreditation standards.
- Conduct routine audits of client charts to ensure compliance with internal policies & procedures (P&P), payer requirements, and regulatory standards.
- Verify completeness and timeliness of: Admission documentation (consents, intake forms, diagnoses), Treatment plans and timely updates, Progress notes (individual, group, psychiatry), Discharge Summaries, Medication Regimens and Client Satisfaction Surveys
- Identify deficiencies that could impact audits, surveys, or licensure reviews and assist in maintaining survey-ready charts.
- Communicates findings of chart audits to Director of Operation and Clinical Leadership.
- Maintains the strictest confidentiality; adheres to all HIPAA guidelines/regulations.
- Other duties as assigned.
Knowledge/Skills/Experiences:
- Utilizing professional methods and techniques of mental health, addictions and dual diagnosis assessment and treatment.
- Knowledge of various mental health, eating disorder, addictions and related illnesses and implement appropriate methods of treatment including proper processes for interviewing and administering assessments.
- Recognizes and is sensitive to cultural and ethnic differences while establishing good rapport with clients and families as well as maintaining effective relationships with the clients and the public.
- Detail oriented.
Qualifications and Education
- Preferred: Two (2) years post-secondary education and experience working in mental health or substance abuse field.
- Experience handling phone intakes, email inquiries and client admissions preferred.
- DHCS Standard 13010(b) (6) requires personnel files to contain health screening reports and tuberculosis test results.
- Computer skills adequate to perform word processing and EHR duties.
