If you're ready to join a team committed to your success, Trilogy is where you belong and thrive!POSITION OVERVIEWThe Master Social Worker (MSW) is responsible for providing psychosocial support to patients undergoing dialysis treatment and counseling families on lifestyle adjustments and coping strategies.Key Responsibilities

Assess patients’ financial, medical, social, and familial needs.
Complete psychosocial assessments and recommend changes in treatment based on the patient’s current needs.
Provide case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease.
Participate in team review of patient progress and completing monthly care plans on patients.
Attend monthly Quality Assurance Performance Improvement (QAPI) meetings and participate in the final development of patients’ Plan of Care on new patients.
Provide pre-transplant counseling for patients and family members and make necessary arrangements with the transplant center.
Provide insurance information on new patients and transients to the business office.
Participate in periodic home visits to complete initial assessments.

Qualifications

Master's degree in social work (MSW) with a specialization in clinical practice
At least two (2) years of experience as a social worker, with one (1) year in a dialysis unit or transplantation program preferred
Must meet the licensure requirements, as required, in the state of practice
