- Assess patients’ financial, medical, social, and familial needs.
- Complete psychosocial assessments and recommend changes in treatment based on the patient’s current needs.
- Provide case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease.
- Participate in team review of patient progress and completing monthly care plans on patients.
- Attend monthly Quality Assurance Performance Improvement (QAPI) meetings and participate in the final development of patients’ Plan of Care on new patients.
- Provide pre-transplant counseling for patients and family members and make necessary arrangements with the transplant center.
- Provide insurance information on new patients and transients to the business office.
- Participate in periodic home visits to complete initial assessments.
- Master's degree in social work (MSW) with a specialization in clinical practice
- At least two (2) years of experience as a social worker, with one (1) year in a dialysis unit or transplantation program preferred
- Must meet the licensure requirements, as required, in the state of practice
