Temporary Housing Case Manager

PsychPros is a nationally recognized staffing and recruiting firm dedicated to the behavioral healthcare field. We are currently hiring a Temporary Case Manager to work full-time with clients experiencing homelessness and staying at an interim housing site in El Monte, CA 91732. This is a TEMPORARY ASSIGNMENT with no set end date and the potential for converting to a permanent position for a strong candidate.

Requirements-

• 2 years of experience in the human services field
• OR a bachelor’s degree in Social Work or in a related field
• Possess a valid California driver's license and have access to properly registered vehicle

Pay-

• $23.50/hour

Schedule-

• Monday-Friday 8:30am-5pm

Responsibilities-

• Conduct assessments of residents
• Determine resident’s need for financial, social, and medical services, and eligibility for services, as well as the resident’s individualized needs & goals
• Evaluate the resident’s mental health and/or substance use related issues
• Develop individual service plans
• Provide appropriate support and referrals to assist with the stabilization of health, finances, and permanent housing needs
• Coordinate intake flow on-site to ensure a streamlined and efficient process for any new move-ins and coordinate site deliveries
• Transport participants as needed to essential appointments and to access necessary community resources
