Overview

Come join our compassionate hospice team as a Medical Social Worker specializing in end of life care. Duties

  • Conduct comprehensive patient assessments to evaluate social, emotional, and environmental needs related to hospice care.
  • Develop personalized care plans that incorporate psychosocial support, counseling, and resource coordination.
  • Provide individual and group psychotherapy sessions utilizing techniques such as behavioral therapy, cognitive behavioral therapy, motivational interviewing, and clinical counseling.
  • Offer crisis intervention and mental health counseling for patients experiencing emotional distress or behavioral health challenges.
  • Collaborate with interdisciplinary teams to ensure seamless case management, including medical documentation, HIPAA compliance, and accurate medical records management.
  • Assist with intake procedures for new patients, ensuring thorough understanding of their social history and care requirements.
  • Support families through grief counseling, addiction counseling, dementia care, Alzheimer's care, PTSD care, and working with individuals with disabilities or developmental disabilities.
  • Utilize EMR (Electronic Medical Records) systems for data collection, documentation of medical terminology, and maintaining detailed medical records.
  • Engage in community outreach and home & community care initiatives to promote hospice services and enhance patient support networks.

Experience

  • Proven experience in hospice care or palliative settings with a strong understanding of end-of-life issues.
  • Background in social work with expertise in child & family counseling, behavioral health, mental health counseling, crisis management, and case management.
  • Hands-on experience working with diverse populations including seniors (geriatrics), pediatrics (including working with children), individuals with autism or disabilities, dementia patients, Alzheimer’s disease patients, and those requiring post-acute or inpatient care.
  • Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding for procedures/services, and medical documentation standards such as HIPAA regulations.
  • Experience using EMR systems for data collection and record keeping is highly preferred.
  • Knowledge of home & community care practices including assisted living environments and outpatient clinics is advantageous.
  • Demonstrated ability to handle crisis situations effectively through crisis intervention skills and crisis management protocols.
  • Strong communication skills for engaging patients in diagnostic evaluation sessions like intake assessments or psychotherapy sessions—both individual and group therapy settings are common components of this role.
