The APF Core Belief

- Lead with Purpose
- Speak Life into Others
- Show Up Fully—Even When It’s Hard
- Model Integrity in All Things
- Inspire Collective Ownership
- Protect the Flame
About the role

Regional Care Managers work directly with individuals experiencing housing insecurity, chronic illness, behavioral health challenges, and justice involvement, helping them navigate care systems, access resources, and achieve stability in health and housing.
