Schedule: Monday – Friday | 5x8 Days (08:30 AM – 05:00 PM)
Location: Washington, DC 20007
Contract: 13 Weeks (Extension Possible)

Job Description (Summary)

We are seeking an experienced Case Manager RN or LMSW/LCSW to join a leading acute care hospital in Washington, DC. This role focuses on inpatient case management, discharge planning, and utilization review to ensure patients receive appropriate care while maintaining compliance with hospital and regulatory standards.

The ideal candidate will have strong hospital case management experience, the ability to work independently with a full caseload, and expertise in care coordination, discharge planning, and utilization management.

This is a weekday-only schedule with no weekends or on-call, making it an excellent opportunity for professionals seeking a stable hospital-based case management role.

Responsibilities

  • Manage inpatient case management services for adult and geriatric patients
  • Coordinate discharge planning and post-acute care services
  • Perform utilization review and medical necessity assessments
  • Collaborate with physicians, nurses, social workers, and interdisciplinary teams
  • Facilitate care coordination and transition planning
  • Ensure compliance with CMS guidelines and Joint Commission standards
  • Support length of stay management and DRG optimization
  • Arrange DME, home health, rehab, and community resources for patients
  • Maintain accurate documentation within the hospital EMR system
  • Participate in interdisciplinary care rounds and case conferences

Requirements / Must Have

Active DC RN License OR DC LMSW/LCSW License (Required at submission)
Minimum 2 years of hospital case management experience
✔ Experience in acute care hospital setting
BLS Certification (Preferred)
✔ Strong knowledge of discharge planning and utilization review
✔ Ability to manage a full case management assignment independently within 1–2 weeks
✔ Experience working with adult and geriatric patient populations
✔ Strong communication and interdisciplinary coordination skills

Additional Details

  • Schedule: Monday – Friday | 8:30 AM – 5:00 PM
  • No weekends, call, or float required
  • First-time travelers accepted with relevant experience
  • Facility Census: ~360 patients
  • Orientation: 28 hours (non-billable orientation)
  • Attire: Scrubs or business casual permitted
  • Parking: Off-site parking available
  • Flu Vaccine: Required (no exemptions)
  • COVID Vaccine: Not required
