Are you passionate about Mental Health? Central Plaza is seeking a part-time Mental Health Professional (MHP) specializing in severe mental illness and substance use treatment and currently looking for part-time position on the 2nd shift (3:00 pm-11:30 pm) and overnight shift(11:00pm-7:30am). The MHP must have at least a Bachelor's degree in a social service field and one year of supervised clinical experience. Our facility is located on the Westside of Chicago in the Austin Area.

Job Summary

As a Specialized Mental Health Rehabilitation Facility, Central Plaza provides Recovery-oriented, person-centered mental health services to ambulatory adults living with symptoms of mental illness and free of medically complex health conditions. Our facility provides holistic services with interdisciplinary teams consisting of social workers, mental health professionals, nurses, and other direct-care staff.

We offer a variety of programming, activities, therapeutic services and 24-hour supervision. Our residents are working on gaining and maintaining mental health stability and developing independence. Many move out of our facility and transition to independent living with community support.

Duties and Responsibilities include:

Oversee the specialized substance use programming for residents and provide case management and counseling services for residents with substance use treatment goals. Manage the development and effective implementation of the substance use programming resident’s Individualized Treatment Plan including, but not limited to, the following:

  • Developing and facilitating psychosocial programming and activities for residents with mental illness who have identified goals related to substance use issues
  • Assess and evaluate the resident’s Recovery interests and needs
  • Develop, review, and modify the Treatment and Recovery Plan in collaboration with the resident and other Team members
  • Provide one-to-one counseling with residents
  • Make recommendations and referrals (both internally and in the community) for services identified on the Treatment and Recovery Plan
  • Help residents learn about and access community resources and programs; provide linkage as needed and encourage participation
  • Track and document resident's progress toward goals and participation in programming
  • Assist residents with discharge planning and help them prepare for transition
  • Collaborate and communicate with nurses, doctors and other team members

Job Type: Part-time

Pay: $23.00 - $26.00 per hour

Expected hours: 18 – 24 per week
