Asbury Village is recruiting for a hospitality focused Resident Care Coordinator to join our team!

GENERAL SUMMARY:
The Resident Care Coordinator is responsible for assisting the Home Services Manager to identify, coordinate, complete, document and market the home services program. The Resident Care Coordinator is responsible for managing the PCA staff schedule to ensure staffing levels are sufficient to provide quality care and services. The Resident Care Coordinator is responsible for assisting PCA staff to find solutions, to understand expectations and to meet those expectations with excellence.

ESSENTIAL JOB DUTIES:
1. Assists with the preparation and management of PCA staff schedules to be approved by the Director to assure consistent and sufficient staffing to meet Residents’ needs.
2. Assumes on call duties as directed by the Director
3. Provide direct care services to residents within the scope of the Personal Care Attendant job description.
4. Assists with the orientation of new PCA staff as directed by the Director
5. Observes Resident/client functioning and notifies the Director and/or Health and Wellness Navigator of observed needs and/or concerns.
6. Provides care and services according to Resident/client’s home service plan of care.
7. Completes appropriate documentation of service provision.
8. Communicates the Resident/Client’s response to home service plan of care to the appropriate clinical leader.
9. Promotes the Home Services Program through educational offerings, marketing/sales visits, other activities.
10. Actively participates in appropriate problem-solving activities keeping the Director informed of any concern that may impact the well-being of a resident, family understanding, Department policy, and regulatory compliance.
11. Encourages a positive relationship between residents, family, and staff by responding to needs in a timely manner and providing information/education as indicated.
12. Participates in quality review activities, summarizes data, and suggests recommendations for improvement to the Director.
13. Keeps the Director informed of all significant matters at the time of occurrence.
14. Assists with presenting staff in-service programs.
15. Demonstrates initiative in any matter that will promote the improved quality of services to our customers.
16. All other duties as assigned.

QUALIFICATIONS:

1. Illinois certification as a Nursing Assistant, LPN, or Medical Assistant
2. Current CPR certification
3. A minimum of five (5) years' experience working with the elderly in a caregiving environment preferred.
4. Previous supervisory and scheduling experience desired. Supervisory skills involving good communication
5. Scheduling experience a plus. Good organizational abilities.
6. Experience leading co-workers, good problem-solving and CQI abilities
7. Familiar with established protocols and regulations in order to assist management operations ensuring accuracy and delivery of services.
8. Job responsibilities typically require considerable judgment and problem-solving in the interpretation and analysis of many diverse facts and issues and the application of appropriate policies, guidelines, clinical protocols, and/or precedents. Ingenuity and foresight are often required in the absence of established guidelines or procedures.
9. Valid driver’s license required.

PHYSCIAL REQUIREMENTS/WORKING CONDITIONS:
Have the ability to move throughout the facility promptly, especially in emergency situations. Must be able to lift up to 50 lbs. unassisted, and up to 150 lbs. with the assistance of one. Must
be able to stoop, bend, and reach; must be able to push and pull up to 150 lbs. in a wheelchair; good visual and hearing acuity required; Good verbal communication skills required. Must be able to walk or stand for up to 80% of the workday. Must be free of communicable disease; may be exposed to communicable diseases, and blood and body fluids that may contain HIV and/or HBV. May be exposed to cleaning chemicals and offensive odors.
Working the majority of time in well lighted, comfortable and ventilated rooms. Frequent stooping, reaching, standing, walking, pushing and pulling. Work is of medium demand and requires consistent lifting up to 10 lbs., frequent lifting up to 20 lbs. and occasional lifting over 75 lbs. Near visual acuity to read documents, measure devices/gauges, and operate other machines. Performs tasks requiring independent knowledge in addition to procedures or instructions provided. Worker subject to both inside and outside environmental conditions. Potential for exposure to blood borne pathogens.
