Social Worker Assistant

Qualifications: Must have a Bachelor's Degree in social work, psychology, gerontology or counseling.

Schedule: M-F 8:30 AM- 3:00 PM.

Basic Function: Under the direct supervision of the Medical Social Worker, the Social Worker Assistant shall provide medical social services and all documentation will be countersigned by the MSW.

Job Responsibilities:

• Participation in initial and continuing assessment of participant’s needs for the development of the individual plan of care by the multidisciplinary team.
• Provide referral to outside resources and assist with counseling when applicable.
• Assist in promoting peer group relationship through problem-centered discussion groups and task oriented committees among the participants.
• Serve as a liaison with the participant’s family and home.
• Serve as liaison with other community agencies who may be providing services to a participant and work with these agencies to coordinate all services delivered to meet the participant’s needs and avoid duplication. Liaison shall include, but not limited to the following agencies:
• In-Home Supportive Services.
• Home Health Agency Providers.
• Other duties as assigned by Program Director.
