Job Overview
We are seeking a dedicated and detail-oriented Intake Coordinator to join our dynamic team. The Hospice Intake Coordinator serves as the first point of contact for patients, families, and referring providers initiating hospice care. This role is responsible for gathering and verifying referral information, ensuring timely and accurate documentation, coordinating admission logistics, and promoting smooth transitions into hospice services.
Key Responsibilities:
Referral & Intake Management
- Receive and process hospice referrals from hospitals, physicians, facilities, and family members
- Obtain, review, and verify patient information, insurance coverage, and eligibility for hospice services as well as obtain authorization for Medi-Cal patients.
- Communicate with referral sources to clarify or complete necessary documentation
Documentation & Coordination
- Ensure timely completion of all required intake and admission paperwork in accordance with CMS, state, and organizational regulations
- Coordinate scheduling of nurse evaluations, hospice admission visits, and DME (durable medical equipment) delivery as needed
- Collaborate with interdisciplinary team members (nursing, social work, chaplain, medical director) to ensure appropriate and timely admission
Communication & Customer Service
- Provide compassionate, accurate information to patients, families, and caregivers regarding hospice services and benefits
- Act as liaison between referral sources, patients, families, and internal departments
- Maintain consistent follow-up and communication throughout the referral-to-admission process
Compliance & Quality
- Ensure documentation meets regulatory standards (e.g., Medicare Conditions of Participation)
- Maintain HIPAA compliance and protect patient confidentiality
- Track referrals and admissions data; assist in reporting and performance improvement initiatives
Qualifications:
- High school diploma or GED required; associate's or bachelor's degree in healthcare administration, nursing, or related field preferred
- Prior experience in hospice, home health, medical intake, or healthcare customer service preferred
- Strong knowledge of hospice eligibility criteria, insurance verification, and medical terminology
- Excellent organizational and communication skills
- Proficiency in EMR systems and Microsoft Office
- Minimum 3 years’ experience required
