A Residential Case Manager establishes supportive, professional relationships with program staff, campus residents, stakeholders, and families; ensuring quality of care, program implementation, regulatory compliance, and case management. This person advises residents in assessing individual needs, setting goals/monitoring progress, and advocating for residents’ access to community resources and opportunities in support of the agency’s vision.
Case Managers make a difference every day by promoting a safe and nurturing environment for campus youth, so they become comfortable with daily routines, feel accepted and valued, and begin to build trusting relationships. In six cottage homes, youth ages 5 to 20 are grouped accordingly to foster a supportive and age-appropriate environment.
Guided by CARE, an evidence-based, trauma-informed model, we create conditions for change where children can heal with help from understanding adults.
As part of a collaborative team of committed staff, you will teach life skills and developmental competencies, provide guidance for educational and career opportunities, and participate in permanency planning for residents.
KEY DUTIES & RESPONSIBILITIES:
· Collaborate with DSS Social Workers, guardians ad litem, and other professionals or vested parties to promote a holistic team approach in the service of residents.
· Participate in the intake process through collaboration with the Residential Services Manager to review and select candidates for admission.
· Secure all required information, documentation, and signatures upon acceptance into the program and at the time of intake/discharge.
· Responsible for measuring outcomes, record keeping/review, and maintenance of all case files with up-to-date information as specified by state licensing and accreditation standards.
· Ensure access to counseling and appropriate services for residents and monitor schedule of health care appointments to comply with licensing and accreditation standards.
· Assist residents with self-assessment of strengths and needs to create and/or update the individual service plan, set goals, and evaluate progress.
· Cultivate caring, supportive relationships with residents and provide a minimum of one hour per month of individual face-to-face time with each resident.
· Participate in ongoing training and development opportunities to enhance your professional skill set and meet training hour requirements for your position.
· Demonstrate knowledge and application of the CARE model and TCI system in your daily practice as a human services professional.
· Assist in planning social, recreational, and enrichment activities for residents that teach competencies and life skills.
QUALIFICATIONS:
· Bachelor’s degree in social work a bachelor’s degree in a human services field and 2 years of related experience an equivalent combination of education and experience
· Valid driver’s license and acceptable safe driving record must be maintained
· Intermediate computer skills
· CARE and TCI certification or willing to become certified through the agency
· 1st Aid, CPR certification or willing to become certified through the agency
