Intake Coordinator (Home Health)
The position requires initiative, problem solving abilities and strong interpersonal skills. You will need to have the ability to multi-task, coordinate new patient referrals, run insurance eligibility, approving and denying referrals and constant follow-up calls and emails.
Responsibilities:
- Conduct comprehensive assessments of patients; medical history, current condition, and treatment needs when in-coming referrals, re-admissions and resumption of cares come in through the fax / email.
- Respond within 5 minutes to our referral sources and verify coverage via insurance carrier
- Obtain proper documentation for clinical review and coverage.
- Collaborate with healthcare professionals, including doctors, physicians, case managers, discharge planners, nursing facilities, etc.
- Advocate for patients rights and ensure that they receive appropriate medical treatment and support while confirming staffing, medication delivery time, start of care, data entry referral, verify accurate PCP and upload all necessary documents onto the patient’s chart.
- Complete welcome calls with patients/ family. Always use the 5 W’s (who, what, where, when and why)
- Maintain accurate and up-to-date medical records for each patient from the time we receive the referral up until the Start of Care visit is completed.
- Must have the ability to report to work on time, follow directions, interact effectively with co-workers, accept constructive criticism and follow procedures.
- You will be an extension of sales, and you will be the first staff member to speak to our referral source, you will need to make them feel welcomed, make their experience fast yet efficient and of course a pleasant one as we need to build rapport.
Qualifications:
- High School Diploma or equivalent required
- Must be task oriented and computer organized
- Proficiency in office skills, including typing and computer skills, professional dialogue while providing customer service with a smile.
- Minimum of two years of experience in a Home Health office preferred
