At Bridges Public Guardianship Program, we pride ourselves on hiring and retaining staff with diverse backgrounds who come from all fields and bring that experience to their work as a Care Manager. This role will require you to advocate for those in our state who have no voice due to being legally incapacitated; in this specific position, these individuals will have challenging mental health diagnoses. These individuals will need your oversight in securing residential placement including coordinating discharge from State Hospitals and communicating with anyone involved in their care on their behalf from doctors, to nursing homes, to group homes, to lawyers, to case workers, and sometimes family members, should they still be involved.

  • The job is largely remote and can be done from anywhere aside from monthly in-person client visits and monthly filing of paperwork at our central office in Fredericksburg, VA.
  • We require a minimum of a high school diploma/GED, though higher education and experience will be considered when hiring for this position.
  • A functioning vehicle and comfort driving is a must. Depending on where you live, you may have to drive quite a distance to reach your clients.
  • This particular caseload is currently largely in the Richmond, Petersburg, Hopewell area.
  • We cover Northwest Virginia, Northern Virginia, Central Virginia, Southern Virginia, and the Tidewater Region. We try to keep caseloads as regional as possible.

Familiarity with Virginia's State Guardianship Program is a MAJOR plus, but not a necessity. This job can only be learned from experience, but any prior exposure to medical, legal, or social work will be a great asset to anyone starting this role.

While you are required to be on call at all times, you have great flexibility in your daily working schedule. We have monthly deadlines for paperwork, but other than that you can make your own schedule. You are asked to visit everyone on your caseload (usually around 20-23 people) at least once per month in person and check in with them at least twice per month by phone and to write notes for each encounter.

We thrive in group problem solving sessions, so we hope that anyone entering this position has experience bringing their questions and ideas to the table whenever they may occur.
