The program offers thorough, strength-based assessments and crisis management to ensure the safety of the youth and others involved. Families, community members, and providers can reach out for support regardless of whether the youth is currently receiving services from Youth Villages.SCS staff work closely with the youth, their family, and key community partners to evaluate risk factors and develop an immediate plan for stabilization. Services are provided in the home or community to offer timely, accessible support during a crisis. In addition to crisis assessment, the program provides ongoing treatment and intervention to help maintain family stability until appropriate long-term services can be secured.Position Overview:The Care Coordinator supports youth who have experienced a mental health crisis event by delivering high quality wraparound crisis care and ensuring the process remains family driven and youth guided. This position carries a shared caseload and provides direct face-to-face support, resource linkage, and coordination of family team meetings. The Care Coordinator develops and monitors Individual Service Plans and collaborates closely with families, natural supports, and community providers to promote progress and stability. This role ensures consistent communication, facilitates transitions, and helps families build the skills and connections needed to successfully navigate services quality wraparound driven and youth guided.Essential Duties and Responsibilities:The Care Coordinator:Ensures wraparound activities are completed at the highest level of qualityCompletes initial evaluation, develops the Individual Service Plan (ISP) with the family, and completes transition plan and/or discharge summaryProvides face-to-face direct service hours to families as needed in implementing wraparoundCollaborates with the family in developing a list of individuals they want to attend the family team meetingsEnsures wraparound process is family-driven and youth-guidedEnsures families experience ongoing participation in and ownership of their care plansFacilitates the scheduling of family team meetings in collaboration with the familyFacilitates the family team meetings and communicates with natural supports and team members regarding youth/families’ participation in services and progress in completing goalsProvides linkage to community resources, clinical services, and natural supports related to the child/youthCoordinates service delivery and meets weekly with family/youth (or as needed per family request)Works with community engagement staff to identify and engage with local community resourcesMaintains and builds trusted relationships with clients, partners, and stakeholdersAttends 3 weekly meetings (individual, team, and clinical consultation) for professional development and quarterly booster training sessions to enhance clinical skillsCompletes accurate and timely documentation in an electronic medical record system (EMR)Performs other duties as assignedAdditional Information:Schedule is flexible and non-traditional as it is based around the availability of youth and families served.Applicants must possess a current, valid driver’s license, an automobile for work purposes, and proof of auto insurance.Community-based staff will be reimbursed for applicable mileage.Skills and Knowledge:Ability to display effective communicationBe approachable and engagingAbility to build teams that are cohesive and promote trustDisplay a knowledge of child-serving systems and local resourcesAbility to encourage youth and family voice and choiceBe knowledgeable and sensitive to the culture of the youth and family and effectively convey that to team membersAbility to empathize with familiesFocus on empowering children/youth and familiesDisplay strong organizational skills and be a self-starterStrong desire to help peopleKnowledge of children’s mental, emotional, and social development and well-beingKnowledge of and experience working with juvenile justice, educational, and other child-serving systemsKnowledge of how to develop family-driven and strength-based treatment/behavior plans with familiesKnowledge of system of care/wraparound principles/practicesExcellent organizational, written, communication, and problem-solving/creative thinking skillsExcellent collaboration, customer service, and leadership skillsQualifications:Bachelor's degree in a social services discipline (required)Degrees that can be considered include: social work, drug and alcohol education or counseling, psychology, criminal justice, guidance counseling, or marriage and family therapy (others subject to review)Experience navigating the child-serving systems (required)Nonprofit organization experience and work history within child-serving systems (preferred)Experience working with children/youth and families with significant behavioral health needs (required)Ability to provide a strength-based, client-centered approach to service delivery (required)Youth Villages BenefitsMedical, Dental, Prescription Drug Coverage and Vision401(k)Time off:2 week paid vacation (full-time) / 1 week paid vacation (part-time)12 paid sick days per year11 paid holidaysPaid Parental LeaveMileage & Cell Phone Reimbursement (when applicable)Tuition reimbursement and licensure supervisionGrowth & development through continuous trainingClinical and administrative advancement opportunities*Benefits are excluded for variable status employees.Youth Villages is an equal opportunity employer and provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.Youth Villages is committed to not only advancing diversity, equity, and inclusion in the workplace but also in our work with children and young adults.
