Care Manager, Complex & Disease Management (DSNP Programs) - Remote

Experteer Overview In this role you provide care management within a multi-disciplinary team to assess complex members, identify gaps in care, and coordinate interventions. You support members to manage health, psycho-social needs, and benefits, enabling safe, independent living. You work across clinics, centers, and homes to improve outcomes and optimize resource use, engaging with diverse stakeholders and ensuring compliant processes. You will drive population health efforts, care transitions, and high-quality care delivery through collaborative, member-centered strategies. Compensation / Benefits • Assess and coordinate care for highly complex members with multidisciplinary input • Identify goals, plans, and interventions including referrals and community resources • Develop and evaluate medical management strategies with members, caregivers, and providers • Engage with the member's PCP and treatment team to set attainable outcomes • Coordinate telephonic and face-to-face case conferences and action plans • Align member needs with appropriate care team members (dietitian, social worker, etc.) • Advocate for the member and facilitate interventions with providers and agencies • Ensure compliance with federal/state/NCQA standards and document in EMR • Maintain knowledge of care management principles and participate in committees and training • Monitor and support ongoing care coordination and plan of care implementation Tasks • RN license, active • 4 - 6 years of clinical experience • Bachelor's degree • CCM certification preferred • Certification in utilization or care management preferred • Strong communication skills (verbal, written, presentation) • Motivational Interviewing training • Experience with electronic medical records • Computer proficiency: MS Office; mobile technology • Bilingual ability preferred • Ability to manage a highly complex caseload and flexible schedule Key requirements •
