Job Summary – MDS Coordinator

The MDS Coordinator (RN) is responsible for managing the resident assessment process, coordinating care planning, and ensuring accurate documentation to support quality care and reimbursement. This role oversees MDS assessments, care plans, and the coordination of services throughout the continuum of care.

Requirements

• Active Registered Nurse (RN) license in the State of Illinois
• MDS experience in a skilled nursing facility preferred (1+ year), but not required
• Knowledge of resident assessments and care planning processes

Pay: $93,000.00 - $95,000.00 per year
