PROGRAM: CCBHC (Certified Community Behavioral Health Clinic)
POSITION TITLE: Care Coordinator
REPORTS TO: Clinical Manager

POSITION SUMMARY:
The Care Coordinator ensures timely, comprehensive, and person-centered care coordination for individuals served by the CCBHC. This role bridges behavioral health, physical health, and social services, supporting continuity of care and accessibility.
KEY RESPONSIBILITIES:
Workflow: Intake → Care Plan → Coordination → Monitoring → CQI/Reporting
1. Access & Intake - Conduct initial client contact and eligibility verification. Maintain accurate waitlist and appointment tracking. Assist with same-day/next-day access initiatives. Support clinicians in auto-assigning clients to available providers.
2. Comprehensive Care Coordination - Develop and update individualized care plans (ICPs). Conduct internal and external referrals (primary care, psychiatry, housing, benefits, social services.) Monitor follow-through and service completion. Facilitate warm handoffs and cross-system collaboration.
3. Client Engagement & Support - Maintain regular contact with clients and caregivers. Provide education, resources, and advocacy. Address barriers to care, including transportation, language, and scheduling. Support culturally responsive and trauma-informed care.
4. Monitoring & Quality Improvement - Track metrics related to access, engagement, and outcomes. Enter and maintain accurate documentation in EHR (DrCloud.) Participate in care coordination meetings and multidisciplinary case reviews. Support CQI initiatives, identifying gaps and recommending workflow improvements.
5. Compliance & Reporting - Ensure documentation meets medical necessity, billing, and audit requirements. Participate in site visit preparation and internal audits. Maintain adherence to CCBHC standards, federal, state, and local regulations.
6. Supervisory Responsibility – None directly; collaborates with peers and reports to Clinical Manager.
QUALIFICATIONS:
Education & Licensure - Minimum: Bachelor’s degree in Social Work, Behavioral Health, Public Health, or related field. Certification in Community in Mental Health, care coordination, case management, or equivalent preferred.
Experience - Minimum 3 years in behavioral health, patient care coordination, or case management. Experience with SMI, SUD, co-occurring disorders, or complex social needs. Familiarity with electronic health records (EHR).
Skills & Competencies - Strong communication, organizational, and problem-solving skills. Ability to work independently and collaboratively in a multidisciplinary team. Knowledge of community resources and referral networks. Cultural competency and person-centered care approach.
Pay: $52,000.00 - $70,000.00 per year
