The Behavioral Health Care Coordinator is responsible for facilitating introductions and coordinating care between partnering facilities and The Josselyn Center’s comprehensive behavioral health services. This role ensures seamless integration of services including intake, assessment, case management, therapy, psychiatric evaluation, medication monitoring, and supported employment.
Key Responsibilities
- Serve as the primary liaison between partnering facilities and The Josselyn Center to coordinate behavioral health services.
- Manage the intake process, ensuring accurate and timely collection of client information.
- Conduct assessments to determine client needs and appropriate service referrals.
- Coordinate case management activities to support client progress and continuity of care.
- Facilitate access to therapy and psychiatric evaluation services.
- Monitor medication adherence and collaborate with prescribing providers as needed.
- Support clients in accessing supported employment opportunities as part of their recovery plan.
- Maintain clear and consistent communication with clients, families, and care teams.
- Document all interactions and care coordination activities in compliance with organizational policies and regulatory requirements.
Requirements
- Master’s level Health Professional
- LCSW/LCPC Preferred and/or a minimum of 2 years related experience, specifically in crisis care setting
- Three professional references that reflect ability to perform clinical work
- Displays high energy, empathy, and organizational skills daily
- Work flexible schedules, including nights and weekends when necessary
- Experience supporting underserved, transient populations with high behavioral health care needs and co-occurring physical health challenges
- Experience identifying and evaluating behavioral health disorders
Qualifications
- Experience in behavioral health care coordination, case management, or related field.
- Knowledge of behavioral health services including therapy, psychiatric evaluation, and medication management.
- Strong organizational and communication skills.
- Ability to work collaboratively with multiple stakeholders and care teams.
- Familiarity with supported employment programs is preferred.
- Relevant certifications or licensure in behavioral health or social services is a plus.
- Fluent English/Spanish preferred
