The Substance Abuse Counselor is responsible for working with the patients on their caseload to complete assessments, treatment plans, and provide counseling, education, case management and support to clients.
Essential Functions include, but are not limited to the following:
- Follow company Policies and Procedures and Counselor's Code of Conduct.
- Conducts Intakes, Discharge Planning. Initial Assessments and Treatment Plans within time guidelines as required by company policies and applicable federal and state regulations.
- Evaluate Client's appropriate level of care and severity of problems or issues initially and on an ongoing basis.
- Determine clinical necessity for counseling services based on the clinical assessment and evaluation of the client's and provide the respective counseling services.
- Determine clinical necessity for counseling services based on the clinical assessment and evaluation of the Client.
- Provide evidenced-based treatment services.
- New Client Orientation; Rules, Policies, Procedures and Patients' Rights.
- Provide Case Management services, including referrals to community resources, mental health and medical services.
- Accurately record all services in a timely manner.
- Other duties as assigned.
Education/Training and Experience:
- Current CADTP (California Association of DUI Treatment Programs) and/or CCAPP (California Consortium of Addiction Programs and Professionals Counselor Registration or Certification.
- A minimum of 2 years experience in substance abuse, mental health and dual diagnosis treatment and service delivery. Some education may be substituted for experience.
- Bilingual (English/Spanish) preferred.
- Trained in SAGE EHR/KIPU
- ASAM Training
