Lead Care Giver/Care Coordinator(Part Time)

Summary:

The Care Coordinator will focus on meeting Client needs and requirements and maintaining Client relationships by identifying the best caregiver/client match, scheduling care, and ongoing case management.

Resoinsibilities:

• Perform in-home client assessments based on direct caregiving experience and observation
• Provide hands-on personal care support as needed, including assistance with ADLs (bathing, dressing, grooming, toileting, mobility, and feeding)
• Develop, implement, and revise individualized care plans using both clinical coordination and practical caregiving insight
• Coordinate and schedule caregivers to ensure consistent, high-quality in-home care services
• Serve as a primary point of contact for clients, families, caregivers, and healthcare providers
• Monitor client well-being through direct care, caregiver supervision, and regular follow-ups
• Identify changes in client condition through hands-on care and communicate concerns promptly to appropriate professionals
• Train, mentor, and support caregivers using real-world caregiving knowledge and best practices
• Step in to provide direct caregiving support during staff shortages or high-need situations
• Ensure care delivery complies with agency policies, safety standards, and state regulations
• Maintain accurate documentation of care plans, caregiving activities, and client progress
• Address client or family concerns with empathy and practical problem-solving
• Coordinate medical appointments, therapy services, and community resources as needed

Job Type: Part-time

Pay: $20.00 - $22.00 per hour

• High school or equivalent (Preferred)
