Location: Bronx, NY (10461)
Schedule: Monday – Friday | 10:00 AM – 6:00 PM
Employment Type: Full-Time | 40 Hours per Week
Compensation:
Travel Rate: $2,647 per week
Local Rate: $2,250 per week
Position Overview
"Navitas Healthcare, LLC" is seeking an experienced and highly organized Registered Nurse (RN) Care Manager to support care coordination within a fast-paced Emergency Department environment. This role plays a critical part in managing patient flow, utilization review, and discharge planning while ensuring high-quality, cost-effective patient care.
Key Responsibilities
Coordinate patient care and transitions directly within the Emergency Department
Conduct utilization review to ensure appropriate level of care and resource use
Develop and manage complex discharge plans in collaboration with interdisciplinary teams
Communicate effectively with providers, patients, families, and support services
Ensure accurate documentation and compliance with regulatory requirements
Required Qualifications
Experience: Minimum of 5 years of clinical Registered Nurse experience
Core Expertise: Care Coordination, Utilization Review, and Discharge Planning
Licensure: Active and unrestricted New York State RN License
Certifications:
BLS (Required)
PRI and Screen Certification (Required within 3 months of hire)
Education: Associate Degree in Nursing (ASN/ADN) required; BSN or MSN preferred
Technical Skills: Strong computer and documentation proficiency
Preferred Qualifications
Prior leadership or supervisory experience
Case Management Certification (CCM)
