Requirements to apply

  • Active CNA and California Home Health Aide (HHA) certification
  • CPR and First Aid certification from the American Heart Association
  • 1+ year of experience working with frail or elderly adults
  • Valid California driver’s license
  • Basic computer skills (Microsoft Outlook, Excel)

Full-time Certified Nursing Assistant (CNA) position within a community-based PACE (Program of All-Inclusive Care for the Elderly) program, responsible for coordinating unskilled home care services, conducting in-home safety evaluations, and supporting care delivery for frail, medically complex seniors living independently in the community. ________________________________________

Job Overview

As a key member of the interdisciplinary team, the PACE Home Care Coordinator (CNA/HHA) provides essential support to ensure safe and compassionate home-based care for participants. This role is responsible for coordinating services like meal deliveries, caregiver visits, and medical equipment drop-offs, while also conducting home safety evaluations and supporting care planning. You’ll advocate for participant needs, ensure services align with care goals, and collaborate closely with nurses, social workers, and other team members to improve outcomes for seniors receiving care at home.
