Position Summary
We are seeking a Medical Social Worker (MSW) to join our Home Health and Hospice team. The MSW plays a vital role in supporting patients and families by addressing the emotional, social, and resource needs that accompany illness, recovery, and end-of-life care. This position requires both clinical expertise and a compassionate, patient-centered approach.
Key Responsibilities
- Conduct psychosocial assessments and develop individualized care plans.
- Provide counseling, emotional support, and crisis intervention for patients and families.
- Assist patients and families in understanding and coping with illness, treatment, and life changes.
- Coordinate community resources, financial assistance programs, and other supportive services.
- Collaborate with the interdisciplinary team (physicians, nurses, aides, chaplains, therapists, etc.) to ensure holistic care.
- Participate in case conferences, care planning meetings, and documentation in compliance with Medicare, state, and agency requirements.
- Provide education and advocacy for patients’ rights, end-of-life decisions, and advanced care planning.
Qualifications
- Master’s Degree in Social Work (MSW) from an accredited program.
- Licensed Clinical Social Worker (LCSW)
- Minimum of 1 year experience in medical, home health, or hospice social work (preferred).
- Strong communication and counseling skills with the ability to support patients and families during stressful times.
- Valid California driver’s license, car insurance, and reliable transportation.
- Ability to work independently, manage time effectively, and coordinate care across multiple disciplines.

Pay: $85.00 - $100.00 per hour
