Pay Rate:
$23.00/hr

Duties & Responsibilities:
The Veteran Directed Care (VDC) Coordinator (Bilingual) - Temporary serves as a liaison between Independent Living, Inc., the agency’s system of care, Veteran program participants, their Authorized Representatives, Care Attendants (CAs), and the Department of Veterans Affairs (VA). This role requires strong organizational skills, proactive communication, and detailed knowledge of VDC procedures to ensure program effectiveness and compliance.

Key Responsibilities:
  • Acts as the primary point of contact for eligible Veterans, their Authorized Representatives, and Care Attendants, facilitating seamless program access and support.
  • Applies working knowledge of:
  • Veteran Directed Care procedures and policies
  • HIPAA regulations
  • Best practices within the VDC program
  • Initiates contact with new referrals within two (2) business days unless a delay is requested by the Veteran or Authorized Representative.
  • Conducts home visits with new Veterans within five (5) business days unless a delay is requested.
  • Oversees the onboarding process for new Veterans and Care Attendants, providing comprehensive program orientation, reviewing the VDC program manual, intake materials, and program requirements; ensures all associated paperwork is accurately processed and verifies participant understanding.
  • Collaborates with each Veteran and their Authorized Representative to develop individualized budget plans, including weekly care hours, and submits these plans to the VA for approval.
  • Schedules, completes, and documents required monthly calls and quarterly in-person home visits to monitor program effectiveness, troubleshoot issues, and review monthly and year-to-date budget reports.
  • Assists Veterans in placing approved supply orders in alignment with their budget plans.
  • Monitors, reviews, and corrects Care Attendants’ timecards each payroll cycle within the ILI timekeeping system to ensure accuracy.
  • Prepares and distributes timely and accurate monthly spending reports to Veterans, Authorized Representatives, and the VA.
  • Accurately enters Veteran and Care Attendant data into the ILI electronic health record system and ensures timely submission of required documentation to HR and Fiscal departments.
  • Maintains ongoing communication and regular meetings with the VA VDC Liaison on behalf of ILI.
  • Proactively ensures timely follow-up with Veterans, Care Attendants, Personal Assistants, the VA, and internal staff, maintaining clear communication and supporting seamless program operations.
  • Assists with office operations and administrative tasks as needed, including replenishing program literature.
  • Provides oral and written translation services as required.
  • Attends and actively participates in all agency-sponsored meetings and trainings.
  • Performs other responsibilities and training assignments as required by the evolving needs of the agency and the position.

Requirements

  • Bilingual in Spanish and English.
  • High regard for accuracy and attention to detail.
  • Demonstrated ability to manage details and ensure task completion through strong follow-up and follow-through skills.
  • Outstanding organizational and customer service skills with a strong customer focus.
  • Excellent written and oral communication skills.
  • Able to work independently on assigned tasks and collaborate with team members on projects as needed.
  • Willing to share ideas and work closely with manager and team members to achieve program goals.
  • Valid, unrestricted driver’s license and acceptable driving record as required by the agency’s insurance carrier; regional travel required for meetings and program support. Unapproved drivers may use personal vehicles and will be reimbursed at the IRS-approved mileage rate upon submission and approval of a travel reimbursement form.
  • Demonstrated ability to recognize the need for and facilitate connections between consumers and Independent Living and other disability-related services.
  • Knowledge of local, statewide, and national disability-related issues and community dynamics.
  • Possesses a working knowledge of mobile devices and is able to efficiently navigate software systems.
