Overview:
Within the administrative framework of the department and under the supervision of a Clinical Supervisor or Senior Social Worker the Social Work Assistant will carry out designated functions of the social work discipline as related to patient care.
Responsibilities: - Establishes and maintains positive relationships with patients, visitors and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of Bronx-Lebanon Hospital Center and its employees
Elicit patient’s social/emotional problems. Provide reality oriented counseling and/or environmental manipulation.
Conduct informational, activity oriented groups.
Carry responsibility for patient placements and disposition.
Escort patients for interviews and home and field visits.
Collaborate with inter-disciplinary team in providing patient care.
Provide necessary reports, record documentation, and statistics as required.
Perform other activities as required assuring services to patients and the functioning of the Social Work Program.
- Prior experience in a social agency or clinical setting under professional supervision.
A minimum of two (2) years post Bachelor’s Degree experience providing inpatient or outpatient services to medical, surgical. or pediatric patients..
Baccalaureate Degree in Social Work or related field in Human Services (Required)
