Full job description

The Care Coordinator 1 guides and assists participants in attaining initial safety and stability toward supporting self- sufficiency. The Care Coordinator 1 assists in accessing necessary resources, which will equip participants with tools to help them sustain their exit from poverty or provide tools to help them maintain a level of stability with government subsidies. This position provides case management services and builds relationships within the community.

  • Develop specific, individualized plans that serve as road maps for clients to get them to a self-sufficient status where they are working towards self-reliance.
  • Analyze individual outcomes regularly, and continue to problem-solve and research new/innovative ways to engage successfully with participants who do not immediately respond to an offered approach.
  • Research and secure information which may include healthcare, psychological and social factors that contribute to the participant’s situation, and evaluate such information along with participant’s capabilities, to more fully inform the development and ongoing assessment of the individualized plan.
  • Counsel participants regarding plans for meeting needs, and aid participants in mobilizing inner capabilities and community resources for improving social functioning.
  • Assist participants in securing entitlements such as SSI, AFDC, Food Stamps, Medical Coupons, State General Assistance, Local Welfare, Child Care and Veteran Benefits.
  • Compile accurate data, prepare accurate reports and conduct follow up procedures utilizing an electronic case management reporting data system.
  • Work collaboratively with team, management and other programs to ensure an overall plan for clients and guests.
  • Other duties as assigned

Schedule: Mon - Fri, 8am - 430pm

  • BA in Psychology, Sociology, Social Work, Mental Health or closely related field preferred
  • AA in Psychology, Sociology, Social Work, Mental Health or closely related field of study required and
  • Two years' experience working closely with individuals and families in need of assistance; understanding the theory and practice of multiple evidence-based and best practices utilized in case management.

OR any combination of relevant education and experience that demonstrates ability to successfully complete essential responsibilities.

  • Knowledge of HIPAA compliance when collecting and storing participant information.
  • Knowledge and ability to access entitlement programs, community, social, medical, mental, chemical dependency services and/or education, job training and employment services within MDC and outside the organization as appropriate based on client(s) need.
  • Knowledge and ability to successfully implement interviewing, counselling methods, principles, and techniques that result in measurable positive outcomes.
  • Ability to analyze interventions for effectiveness, research and develop alternative interventions, openly solicit supervisory guidance and input.
  • Proven ability to maintain accurate, thorough, timely and complete case files and ability to communicate professionally in writing and verbally; Knowledge of electronic recordkeeping for client information.
  • Demonstrated ability to work with difference, specifically around race, ethnicity, religion, sexual orientation and gender identity, disability, etc., and how these differences contribute to disparate outcomes for these populations.
  • Washington State Driver’s License; maybe required to drive company vehicle, and is insurable for purpose of transporting clients to scheduled appointments and/or visiting and monitoring client’s living environment.
