SUMMARY:
The Care Coordinator (CC) is a non-clinical team member who facilitates patient continuity of care with the healthcare team. Under the clinical direction and oversight of the Supervisor of VBC Support, Primary Care Physicians and department clinical staff, the Care Coordinator navigates patients at risk by collaborating with the patient and interdisciplinary healthcare team to serve needs and expedite appropriate, cost-effective care.
The Care Coordinator (CC) will support the clinical and operations teams in the care of patients. They will engage patients and take an active role in improving the health of patients we serve. They will partner with the care team in providing the patient with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, and close gaps in care.
PREFERRED QUALIFICATIONS:
Two (2) + years of work experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management or Home Health Care. High school degree required.
PHYSICAL GUIDELINES:
Physical Guidelines include the ability to move, traverse, position self, remain in a stationary position and negotiate steps for up to eight hours per day.
