The Supervisor of Social Work provides administrative, clinical and supportive supervision to a team of social workers. This position plans, organizes, and directs the work of staff and provides subject-matter leadership to the team, as well as to the organization related to social services and discharge planning. The Supervisor assists with the daily operation of their assigned program such as discharge planning activities and collaborates with medical staff and the multidisciplinary team in developing plans that meet patient/family psychosocial and discharge needs.

WHAT YOU CAN EXPECT
Schedule: Monday - Friday, 8am - 4:30pm
Participate in people management activities for direct team members, such as conducting performance evaluations, disciplinary actions, and interviews.
Oversee documentation procedures, requirements for records and reporting compliance, in accordance with state and federal rules, and regulations and/or accrediting body’s conformance standards.
Supervise the work of others and manages the performance of individuals through feedback and recommendations.
Perform thorough patient psychosocial assessments to explore the physical, psychological and social aspects of the patient and their conditions.
Elicit an accurate description of psychosocial needs, in order to inform patient treatment plans, in collaboration with patients, their support systems and care team members.
Develop discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family, and multidisciplinary care team to make the transition as smooth as possible for patients along the continuum of care.
Integrate insurance, financial situation and criteria for payment in discharge plans in consideration of patient/caregiver needs.
Maintain good rapport with internal and external referral sources; facilitates communication with community facilities, agencies, and resources and makes appropriate referrals as needed.
Participate in and/or gathers key stakeholders to lead System taskforces, committees, or workgroups with clear roles, understandable information, and actionable outcomes to implement changes, resolve issues, and ensure ongoing effectiveness.
Document patient care information and ensures timely and accurate completion of patient care records, in accordance with policies and procedures.
QUALIFICATIONS
Master's Degree (Social Services related) required
4 years of Acute Care Hospital experience preferred
Indiana Licensed Social Worker (LSW) required
National Association of Social Workers (NASW) preferred
