Job Overview
We are seeking a dedicated and compassionate Certified Social Worker to join our Adult Day Health Facility. The ideal candidate will provide comprehensive social work services in our facility located in Passaic, NJ and will work together with our current Social Worker. This role involves supporting patients and their families through assessment, counseling, case management, and coordination of care to promote overall well-being. The Certified Social Worker will play a vital role in delivering patient-centered care in compliance with HIPAA regulations and medical documentation standards.
Duties
- Conduct thorough patient assessments to identify social, emotional, and environmental needs.
- Assist patients with crisis intervention and crisis management to ensure safety and stability.
- Develop personalized care plans and coordinate services for patients with complex needs, including those with developmental disabilities, dementia, Alzheimer’s disease, PTSD, or autism spectrum disorders.
- Document all interactions accurately within Electronic Medical Records (EMR) systems while adhering to HIPAA guidelines.
- Collaborate with multidisciplinary teams to facilitate case management, discharge planning, and care strategies.
- Facilitate intake procedures for new patients and manage ongoing patient care activities such as medical records review and diagnostic evaluations.
- Engage in community outreach programs to enhance patient support networks and access to resources such as assisted living or senior care services.
- Maintain current knowledge of relevant healthcare regulations.
Skills
- Proven experience in adults/senior services.
- Experience working with diverse populations including geriatrics, individuals with disabilities or developmental disorders, and those requiring dementia or Alzheimer's care.
- Excellent patient assessment skills combined with crisis intervention expertise.
- Ability to perform behavioral health interventions such as applied behavior analysis (ABA), cognitive behavioral therapy (CBT), motivational interviewing, and psychotherapy.
- Knowledge of HIPAA compliance, medical documentation standards.
- Effective case management skills with the ability to coordinate complex care plans across multiple providers.
- Strong communication skills for working with patients of all ages.
- Ability to handle sensitive situations involving mental health crises or behavioral health challenges with professionalism and empathy. This position offers an opportunity to make a meaningful difference in patients' lives through compassionate support and expert social work services.

Pay: $34.00 - $38.00 per hour
