Salary: Up to $70,000 per year

The Monarch at Brooklyn Rehabilitation & Nursing Center is seeking a compassionate and dedicated Social Worker to join our interdisciplinary care team. This role plays a vital part in supporting residents and families throughout their stay and ensuring high-quality, person-centered care.

Social Worker Responsibilities include:

• Providing psychosocial assessments and ongoing support to residents and families

• Assisting with discharge planning and coordination of community resources

• Participating in care plan meetings and interdisciplinary team discussions

• Supporting residents’ rights, emotional well-being, and care transitions

• Maintaining accurate and timely documentation in accordance with regulations

Social Worker Qualifications:

• BSW or MSW required

• Long-term care or SNF experience preferred

• Strong communication, organizational, and interpersonal skills

• Knowledge of Medicare, Medicaid, and discharge planning processes
